bullet Alzheimer’s disease bullet Huntington’s Chorea
bullet Hemophilia bullet Cancer (except basal cell skin cancer)
bullet Cerebral Palsy bullet Major Organ Transplantation
bullet Coma bullet Multiple Sclerosis
bullet Cystic Fibrosis bullet Muscular Dystrophy
bullet Dementia bullet Parkinson’s disease
bullet Down’s syndrome bullet Pulmonary Fibrosis
bullet Sickle cell disease bullet Epilepsy (uncontrolled)
bullet Suicide attempt within the past two years bullet Hepatitis B (carrier with current viral activity)
   or C
bullet Blindness – coverage may be available
   with exclusions
bullet Kidney disease (other than kidney stones    and/or a history of kidney infection)
bullet Lupus (other than Discoid Lupus    Erythematosus) bullet Deafness – coverage may be available
   with exclusions
bullet Paralysis (other than Bell’s Palsy) – coverage    may be available with exclusions bullet Progressive motor neuron disease (i.e.,    Amyotrophic Lateral Sclerosis (ALS) –Lou    Gehrig’s Disease)
bullet Drug use within the past three years (other    than social marijuana use) bullet Stroke – Cerebrovascular accident or    Transient Ischemic Attack
bullet  Diabetes
    bullet Insulin-dependent Diabetes mellitus
    bullet Non-Insulin-dependent Diabetes        mellitusunder age 40
    bullet Non-Insulin-dependent Diabetes        diagnosed within the past six months
bullet Heart – any heart condition or heart/cardiac    trouble including: heart attack, angina, bypass    surgery, congenital heart conditions, coronary    angioplasty, heart valve surgery
